I think the insurance fraud episode is way later as the hospital director is played by Monica from friends. But maybe they did that twice and I just forgot?
IIRC it was very early on - JD noticed a surgery scheduled for an empty bed and corrected. Proud of himself he let Kelso know, only to find out later that Cox did it on purpose.
